FMGE vs NExT: Which Exam Will MBBS Abroad Students Have to Clear?

If you're planning to pursue MBBS abroad or are already studying in countries like Russia, Georgia, Kazakhstan, Uzbekistan, or Kyrgyzstan, you've probably heard two terms repeatedly: FMGE and NExT. Many students and parents are confused. Will FMGE continue? Has NExT already replaced FMGE? Which exam should MBBS abroad students prepare for? Here's a complete breakdown to clear your doubts.

What is FMGE?

The Foreign Medical Graduate Examination (FMGE) is a licensing examination conducted by the National Board of Examinations in Medical Sciences (NBEMS). Indian citizens and Overseas Citizens of India (OCI) who complete their MBBS from a recognized foreign medical university must qualify FMGE before they can register to practice medicine in India.

Key Features of FMGE

  • Applicable only to Foreign Medical Graduates
  • Conducted twice every year
  • Computer Based Examination
  • 300 Multiple Choice Questions
  • Passing marks: 150 out of 300 (50%)
  • Required for registration with the National Medical Commission or State Medical Councils after meeting all other eligibility requirements

What is NExT?

The National Exit Test (NExT) is a proposed national examination introduced under the National Medical Commission Act. The long-term objective of NExT is to create a single common examination that will:

  • Grant a license to practice medicine in India
  • Serve as the entrance examination for postgraduate medical courses
  • Eventually replace FMGE for foreign medical graduates once implemented

Unlike FMGE, NExT is designed to evaluate both theoretical knowledge and practical clinical skills.

Has NExT Replaced FMGE?

No. This is one of the biggest misconceptions among MBBS abroad aspirants.

⚠️ As of now, FMGE continues to be the official licensing examination for foreign medical graduates
Although NExT has been proposed as a replacement, its nationwide implementation has been postponed. Reports indicate that the National Medical Commission plans mock implementation and phased testing before introducing it fully. Until an official notification is issued, students should continue preparing according to the existing FMGE system.

Why Was NExT Introduced?

The Government aims to create a uniform evaluation system for all medical graduates. Currently:

  • Indian MBBS graduates complete their university examinations and then appear separately for postgraduate entrance examinations
  • Foreign medical graduates appear for FMGE before becoming eligible for registration

The proposed NExT system seeks to combine licensing and postgraduate eligibility into one common framework, making medical assessment more standardized.

Which Examination Is More Challenging?

FMGE primarily tests theoretical medical knowledge through multiple-choice questions. The proposed NExT framework is expected to assess:

  • Clinical reasoning
  • Practical decision making
  • Patient management
  • Medical knowledge
  • Clinical skills
💡 This means NExT is expected to evaluate not just what students know, but also how effectively they apply that knowledge in real clinical situations
Making it a more comprehensive assessment of medical competence.

Why Is FMGE Considered Difficult?

Every year, thousands of foreign medical graduates appear for FMGE. Recent sessions have shown relatively low pass percentages, highlighting the importance of strong conceptual preparation and consistent revision. Some common reasons include:

  • Large syllabus
  • Concept-based questions
  • Limited clinical exposure at some universities
  • Inadequate preparation after graduation
🎯 Students planning MBBS abroad should consider these factors while choosing a university
Choose a university that provides strong clinical exposure and conceptual teaching.

What Should You Do?

If you are planning to study MBBS abroad:

Choose a university that complies with National Medical Commission regulations regarding course duration, internship, and eligibility requirements.

If you are already studying abroad:

Continue focusing on building strong medical concepts. Do not postpone your preparation waiting for NExT. FMGE remains the applicable licensing examination until official changes are announced.

Final year students:

Stay updated through official notifications from:

  • National Medical Commission (NMC)
  • National Board of Examinations in Medical Sciences (NBEMS)
⚠️ Avoid relying on social media rumours regarding examination changes
Always verify information through official sources.

FMGE or NExT: Which Should You Prepare For?

The answer depends on the official regulations in force when you complete your medical degree. As of today:

  • FMGE is the operational licensing examination for foreign medical graduates
  • NExT has been proposed as the future unified examination but has not replaced FMGE yet
📌 Students should therefore prepare according to the current rules while keeping themselves informed about future developments
Stay updated through official NMC and NBEMS notifications.

Final Thoughts

The discussion around FMGE and NExT has created confusion among many students planning MBBS abroad. The important thing to remember is that nothing changes until an official notification is issued by the concerned authorities.

At present, FMGE remains the pathway for foreign medical graduates seeking registration in India. Whenever NExT is introduced nationwide, it is expected to create a more uniform licensing system for both Indian and foreign medical graduates.

Until then, students should focus less on speculation and more on building strong medical knowledge, choosing the right university, and preparing consistently for the licensing examination that is currently applicable.
